Candidates must use specific multiple-choice answer sheets for Cambridge IGCSE and O Level foreign language, English as an Additional Language, and Mandarin Chinese listening exams. We pre-print these with candidate name and number and send to you. Read more about these forms and see examples.
Please note: From the March 2024 series we are introducing additional multiple-choice answer sheets. The multiple-choice answer sheet a candidate must use depends on which syllabus they are sitting. There will be three more versions of the multiple-choice answer sheet available, and candidates must use the correct version. This form is to be used by the Head of Centre to report suspected candidate malpractice. This includes suspected candidate malpractice in timetabled exams or other assessments, apart from instances relating to unauthorised materials. Please use Exam Day – Form 9d to report instances of suspected candidate malpractice involving unauthorised materials.

Use this form to report instances where candidates have brought unauthorised materials into the exam room or have accessed unauthorised materials during the exam. This includes timetabled exams or other assessments. Section 5.1.8.2 of the Cambridge Handbook provides examples of unauthorised materials.
